1) Yes the logo should be designed as .svg. Also to get the lines effect the SVG should be “stroke” based:
In this specific Demo Band logo, first we add the class active after 1 second trough the code at “js/demos/demo-band.js”. The active class starts the animation trough CSS at “css/demos/demo-band.css” (lines 34 and 42).
You can also animate stroke based SVG’s trough the data-icon plugin. You can see examples here:

2) This case you should use as base for you contact page, the code at contact-us-advanced.php. Note will be required you run your project trough a online or local server running PHP as the backend language.
