Hello @rcarpi2, you use the same Host I’m using on my Preview Website, GoDaddy, that’s weird it’s not working for you.

I know you said you’ve tried everything, but I have to ask, did you try to replace the PHP file using that one: http://www.okler.net/files/contact-form.zip (and just replace the email)?
Also, are you using an email address from the domain?

If none of this works, just ask to godaddy technical support to enable “sendmail” to make sure it’s enabled.
